As a DBA I too hate documentation as well. The worse problem is many DBA's documentation I read is not standardized so they decided to write a novel. To me this is equivalent to no documentation because there is way too much irrelevant information to comb through. During an emergency this is too much to read and you have no idea if what you need is even included.

I developed several templates after reading your post based on what you discussed. I will present it to the team next week and see if we can standardize on it. Thanks!
